Thank you for volunteering to help test our newly updated, more comprehensive Life Renewed MAPS™ Assessment.

Our revolutionary Life Renewed MAPS™ is a scientifically-based assessment software that measures and tracks spiritual,mental, social, emotional, and physical health perceptions. The spiritual component of the assessment examines the overall health and condition of the human spirit.

You begin by completing our on-line, multiple-choice Life Renewed MAPS™ Assessment. Once your assessment is completed, it is automatically scored and analyzed. Then our software system generates a personalized assessment report. Reviewing the assessment report will help you better understand your functioning and well-being.

Upon submission of your registration you will be emailed the information you need to log on to the Life Renewed MAPS™ Assessment. After receiving the log-on information, return to this page and click on the "Take Assessment" button below.
