Life Renewed™ Quality of Life Assessment™ software assesses these dimensions:
Physical health, physical limitations, body pain, energy level, health behaviors, general health perceptions, emotional/mental health, emotional limitations, social functioning, social compatibility, life experiences, life purpose index, support system, stress index, spiritual beliefs, and an individual’s depression level. When the assessment is completed, it is automatically scored and a personal report generated. Taken at regular intervals, the assessment accurately tracks a client’s progress. The aggregate data from multiple clients’ assessments demonstrates the effectiveness of the program. The assessment report is the blueprint for improving clients, instructors and programs.

The Life Renewed™ Quality of Life Assessment™ software can be downloaded and installed from the internet or compact disc. It has been designed for ages 14 and over and is not gender specific. This assessment can be used as part of the client intake and volunteer screening procedure of your program and can be used with a wide variety of existing programs to measure individual growth, quality of instruction, and program effectiveness. The assessment takes approximately 30 minutes to complete depending on reading level. The multiple-choice questions are easy to understand and answer. This assessment is designed to plug into your program seamlessly and is user friendly.
